THK Home Care Services
THK Home Care Services provide holistic and person centred care to the elderly who are mainly the needy and vulnerable, staying in the community. The focus of our services is to ensure the elderly’s health and Activities of Daily Living (ADLs) are optimized by delaying the onset and/or worsening of chronic illnesses e.g. diabetes, hypertension, dementia with the fine balance of healthy living.
Our objectives are mainly:
- To provide quality home care services to help the elderly and adults with permanent disabilities to live in the community for as long as possible
- To promote autonomy, dignity, self-determination and self-care
- To provide more choices of services – in terms of medical, clinical, home rehabilitation, emotional and social needs
- To reduce the burden and stress of caregivers
- To provide continuity of care:
- Rapport building
- Reduce “Frequent Flyers” and hospital admission
- Reduce anxiety
- Reduce falls and early detection of dementia
Who is eligible?
Any adult with permanent disabilities or elderly who requires assistance with healthcare support in their homes.

INTERIM CAREGIVER SERVICE (ICS)
Interim caregiver service is a home care service for patients (clients) pending discharge from Restructured or Community hospitals. The client may need interim post-discharge support at home while waiting for more permanent care arrangements e.g. nursing home placement or foreign domestic worker (FDW) to arrive.
Services Provided
Our Senior Care Associates (SCAs) who are foreign nurses in their own country will visit the client for a day to build rapport with the client and nurses in the hospital, learning about the care for the client post-discharge. He/she will then accompany the client home and provide either 8 am to 8 pm (day shift) or 10 pm to 8 am (night shift) for 2 weeks (excluding Sundays and Public Holidays). Alternatively, the client can choose to have a 6-day 24 hours shift. The client’s vital signs will be monitored and recorded, intake/output, etc. on a daily basis ensuring that the client is stable. The client will be holistically cared by the SCA who provides personal hygiene, medication administration as taught by the hospital, escort for medical follow-up, etc.
HOME PERSONAL CARE (HPC)
Home Personal Care (HPC) provides basic nursing care to the elderly encouraging self-care so that the homebound elderly can continue to live in the community. This service provides support to the caregivers and is one of the other alternative care options.
Services Provided
Personal hygiene care, light housekeeping, mind simulation activities, monitoring of medication compliance and assistance with other personal care services e.g. nail filing, simple wound dressing, etc.
HOME HEALTH SERVICE (HH) – HOME MEDICAL AND HOME NURSING
We provide professional health care services to the elderly by our licenced doctors and nurses. These services are provided to reduce repeated hospitalisation, caregiver stress, delay admission to health care institutions and improve the quality of life of our clients.
Services Provided
Insertion of tubes and catheters, wound dressings, stoma care, tracheostomy, caregiver’s training, medication review, etc. are provided to the elderly in their own home.
