THK Adult Rehabilitation Programme was set up to bring rehabilitation into the community, bridging the gap between institution and home.
Objectives of the centre:
- To provide sessional day rehabilitation services to all adult persons living within a 5-km service boundary of the centres.
- To engage residents living in the surrounding vicinity of the centre in outreach activities as part of the communities’ health promotion efforts.

Standard day rehabilitation therapy services:
- Physiotherapy
- Occupational Therapy
- Home Therapy Services
Non- standard day rehabilitation therapy services:
- Transport Services available for clients receiving day rehabilitation services
- Speech Language Therapy
Speech Language Therapists (SLT) provide assessment, treatment and management of speech, language, cognitive-communication, and swallowing difficulties. Some of the commonly treated issues are described below.
Speech Disorders
|
- Difficulty with planning and/or producing speech sounds
- Speech may be unclear or difficult to understand
- Can be caused by neurological diseases (e.g. strokes), structural changes (e.g. loss of dentition)
- Following assessment, SLT may recommend rehabilitation exercises, compensatory strategies, exploration of alternative modes of communication, and/or other intervention

Language Disorders
|
- Difficulties with understanding and/or use of language for communication
- Affected individuals may have difficulties finding the correct words to use, understanding and/or using spoken and/or written communication
- May result from damage to parts of the brain responsible for language
- Following assessment, SLT may recommend rehabilitation exercises, compensatory strategies, exploration of alternative modes of communication, and/or other intervention

Cognitive-communication Deficits
|
- Difficulties with thinking which impacts on communication
- Affected individuals may have reduced attention, memory, reasoning, planning and/or social skills
- May result from damage to parts of the brain responsible for cognition
- Following assessment, SLT may recommend rehabilitation exercises, compensatory strategies, exploration of alternative modes of communication, and/or other intervention

Swallowing Difficulties
|
- Difficulties in chewing and/or swallowing food and/or drinks
- Often due to damage or weakness in muscles and nerves used in swallowing
- Affected individual may cough or choke, feel like food is stuck in the throat area, and/or experience food/drinks spilling out of the mouth when eating or drinking
- May be caused by neurological diseases, structural issues, side effects of medical treatment and other medical conditions
- Following assessment, SLT may suggest modification of food/drinks, and recommend compensatory strategies and/or rehabilitation exercises
