Mission 
The mission of Cluster Support is to enable vulnerable seniors with no or low family support to age-in-place and remain integrated within the community for as long as possible.

Objectives

a. To provide social support through case management, monitoring, and basic counselling services to vulnerable seniors within the service boundary assigned by the Government; and

b. To facilitate better coordination in the provision of community-based care and support services to the seniors.

Desired Outcomes

1. Vulnerable seniors receive adequate, timely and co-ordinated care and support; and
2. Vulnerable seniors have access to community resources and social support to live safely and independently in the community.

Target Clientele

The primary target clientele of the CS are residents within the identified service boundary who meet the following criteria:

1. Aged 60 and above
2. Assessed to have no or low family support and need continued assistance and
3. Requires coordination of services for issues relating to emotional support, medical compliance and/or self-care, which includes their ability to carry out their Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).

 For Cluster Support sited at a HDB Studio Apartment block, we also manage:

1. The Alert Alarm System (AAS)
2. A Drop-in Activity Area for Seniors
