Home Care ServicesTHK Home Care Services provide specialised and compassionate home healthcare through various services. Home Health Service encompasses Home Medical (HM) and Home Nursing (HN), addressing specific medical needs. We also offer Home Personal Care (HPC) and Interim Caregiver Service (ICS) for a comprehensive approach. Our dedicated professionals focus on enhancing clients’ quality of life, independence, and dignity at home.

THK Home Care Services provide a comprehensive range of services designed to meet various healthcare needs, ensuring personalized care and support, including:
1. Home Health – Home Medical (HM)
Managing chronic medical problems
Managing uncomplicated acute or sub-acute medical problems
Referring patients to specialists
Arranging safe transfers for hospitalization
Educating clients and caregivers on the client’s illnesses and care plan
2. Home Health – Home Nursing (HN)
Providing nutritional advice
Providing nutritional advice Conducting nursing interventions (wound care, changing feeding tubes, changing indwelling catheters)
Managing medications (medication packing)
Monitoring vital signs
Offering social and emotional support
Providing patient/ caregiver education
3. Home Personal Care

4. Interim Caregiver Service
Suitable for seniors who lack caregiving support / caregiver unable to manage care of senior at home post-discharge
Interim period of support: Up to 2 weeks, 12-hour shifts
Common Scenarios: While awaiting more permanent care arrangements for seniors, such as the arrival of a foreign domestic worker, placement in center-based services, or activation of home-based services.
Service can only be referred by Medical Social Worker from the hospital only, when the senior is still warded.
*Service admissions for Home Health and Home Personal Care services will be assessed based on eligibility criteria.
